The UA Collection Site List was updated 2/11/13 - TWO new sites were added in Castle Rock and Granby.  Make sure you are using an updated list.

Effective May 4, 2013 All Monthly/Quarterly Reports are due on the 4th of the month for the previous month. 

Starting in May 2013 paperwork and reports for all participants in the Peer Health Assistance Programs (PHAP) will be due on the 4^th of the month. 
When you submit your monthly paperwork on May 4^th, it will include the entire month of April (April 1-30).
